To: Dear OSU Community Members in Benton, Linn, and Lincoln Counties

March 30, 2021

Dear OSU Community Members in Benton, Linn, and Lincoln Counties,

We have just been informed that Benton County and Samaritan Health Services’ success in immunizing Groups 1-6 defined by the Oregon Health Authority has enabled the county to move to vaccinating Group 7 front line workers early. Front line workers are defined as:

  • Someone who has a job that puts the individual at higher risk for contracting COVID-19 because of regular close contact with others outside of their household.
  • Someone who has routine close contact with others outside of their household and who cannot perform their job duties from home or another setting that limits close or routine contact with others outside of their household.

We are very pleased that more OSU employees are now eligible to be vaccinated.

If you fit into these employment categories and are interested in receiving a COVID- 19 vaccine, please use the link below to sign up for an appointment this week to be vaccinated at the Benton County and Samaritan Health Services clinic located at Reser Stadium. This link is a Samaritan Health Services service, and your information will not be shared with OSU. If you need assistance with signing up please ask your supervisor or manager.

Reser Mass Vaccination Clinic Schedule

Wednesday, March 31 9 a.m. to 5 p.m.
Thursday, April 1 9 a.m. to 4 p.m.

To link your vaccine application to your electronic medical record, you will need to provide your Social Security number in the scheduling form. If you are not comfortable providing this information, you may enter all zeros. Insurance information is requested but not required and there is no fee to the individual for receiving a vaccine.

If you do not fall into the Group 7 front line workers category, please know that we will keep you updated with information on other vaccination clinics as access to vaccines expands.  If there are no appointments available when you are accessing the link, please check back periodically as appointment slots will be added as more vaccines are allocated locally.

Sincerely,

Dan Larson
